Helen Keller - (1880 - 1968) - Helen Adams Keller (June 27, 1880 - June 1, 1968) was an American author, activist and lecturer. She was the first deaf/blind person to graduate from college. She was not born blind and deaf; it was not until nineteen months of age that she came down with an illness described by doctors as “an acute congestion of the stomach and the brain”, which could have possibly been scarlet fever or meningitis. The illness did not last for a particularly long time, but it left her deaf and blind. Keller went on to become a world-famous speaker and author. She is remembered as an advocate for people with disabilities amid numerous other causes. Stevie Wonder - (born Steveland Hardaway Judkins on May 13, 1950, name later changed to Steveland Hardaway Morris), is an American singer-songwriter, multi-instrumentalist, and record producer. Blind from infancy, Wonder signed with Motown Records as a pre-adolescent at age twelve, and continues to perform and record for the label to this day. It is thought that he received excessive oxygen in his incubator which led to retinopathy of prematurity, a destructive ocular disorder affecting the retina, characterized by abnormal growth of blood vessels, scarring, and sometimes retinal detachment. Franklin Delano Roosevelt - (January 30, 1882 - April 12, 1945) Franklin was the 32nd President of the United States of America and played a big role during World War II. Roosevelt eventually aided the poor and un-employed of America and restored order at various times during his Presidency. He was also the only President to ever get elected 4 years in a row mostly because of his help for the recovery of the economy. It has been said that Roosevelt had several disabilities including vision impairment. Harriet Tubman - (c. 1820 - 10 March 1913) Harriet Tubman was a slave throughout her youth, being treated as an animal until she eventually escaped captivity. When she had reached Canada she did not stay to enjoy her freedom. She returned to the lands and brought hundreds of black slaves back to safety, saving them from slavery by escaping from what they then called The Underground Railroad. After a severe wound to the head, which was inflicted by a slave owner before her escape, she became victim to vision impairment and seizures. Which did not keep her from tossing her fears aside and to keep fighting for the freedom of her people.

You are a medical assistant in a large physician practice that is going to be growing by adding additional physicians. The practice will need to hire and train quite a few new medical assistants to support these new physicians. They have contacted a local college to allow them to post ads in the medical assisting program to recruit new graduates to these positions.The office manager has asked your team to write a sample SOAP note that new hires will be asked to read and interpret. She has also asked that your team write a sample training outline based on the sample SOAP note so that one of your team members can present an in-service education and help new hires correct any deficiencies in reading this SOAP note. Each member of the group should write a brief SOAP note of at least 4 paragraphs, documenting a fictional patient visit and including medical terms with suffixes, roots, and prefixes. The individual member should select either a muscular or skeletal body system patient problem to which the medical terms will refer. Go to the Library and use the web resources to help you develop your SOAP note. Use your knowledge of other medical terms to develop a SOAP note too. For the assessment part of your note, write a summary of the findings rather than a medical diagnosis. For the plan, you can note what medical terms seem realistic to use in terms of follow-up. The goal is to develop a document where the new hire can demonstrate his/her knowledge of medical terminology. When you encounter abbreviations or acronyms that you are unfamiliar with, you may use your own resources or the web resources to determine the meaning of those abbreviations or acronyms. Your individual deliverable should identify acronyms or abbreviations that could be misinterpreted and how you have modified your SOAP note to avoid that possible misinterpretation. For example, one of the acronyms used in one of these SOAP notes refers to the acronym SBE. This can mean self-breast examination, which is also seen as BSE (breast self-examination) or an illness called sub acute bacterial endocarditis referring to an infection of the heart. Obviously these have very different meanings. Your group should select or combine features of the individual SOAP notes into one SOAP note to produce 2 SOAP notes. There should be at least 10 medical terms used, and terms from at least 2 body systems should be mentioned. Remember that patients who visit a certain kind of physician may have current medical problems in other body systems.Your group should develop a training outline that includes major and minor bullets on how to interpret medical terms seen in this sample medical record. Your group will need to consider what topics to include, a behavioral objective for each topic, and how much time should be allocated to each topic. Your group should then produce this training outline as a PowerPoint presentation. Your group will also produce a document of the group’s SOAP note.
